Submit your answers in a Word Document. Be sure to note which chapter you selected and you are required to re-type the questions and underline your answer area.
Any ideas that are not your own must be cited within the body of the answers for each question, must be included in your reference list and the end of your assignment and must be in APA style format. Citing your sources is significant and it must be done properly. This assignment must have 3 sources minimum and must contain critical thought so it should be minimum 2 full PROPER paragraphs per question. Sources are to be cited in paragraphs to verify your answers.
Be aware you must put a header on all homework that includes your name, the assignment, and the class/section. HEADER DOES NOT MEAN JUST PUTTING THIS INFORMATION AT THE TOP OF THE PAPER! You need to open the header, put the information in and close it. If you do not do this on the assignment, you will automatically lose points. Second, all homework must be typed and uploaded. The question first, then the answer which should be underlined. If this is not done properly, it is points off automatically. Finally, you must cite sources used, if they are not cited in APA correctly, it will be points off automatically. If found to be plagiarized or unoriginal for more than 20% then you will get Zero for the whole assignment on first time and warning, second, it will be zero in the course and turned into the academic affairs committee, and third offense removed from the Health Information Management Program. There are very few things that you must type in this class if any, so just be aware.
